Tetanus Toxic, series completed 6 Jan 51 (Booster)  Cholera Vaccine, Series 
Completed 6 Jan 51

Jap B. 6 June, 10 June, 13 June done.

Smallpox Vaccine, 24 Oct 50, 14 Dec 50 and 6 June 51.
